An NHS clinical audit team wanted to investigate junior doctors' experiences and reporting of "near-miss" clinical errors in high-pressure emergency departments. The lead researcher randomly selected four major hospitals within a regional NHS trust. Three of the four hospitals agreed to participate in the study. The researchers designed a structured questionnaire containing six specific questions about witnessed clinical errors and personal levels of fatigue. They administered these questionnaires face-to-face to junior doctors in the communal staff rooms during their active on-call shifts.
