Bovine Leukemia Virus (BLV) is a retrovirus that infects beef and dairy cattle. BLV binds to a primary receptor called CAT1 on bovine B-lymphocytes. Current management of BLV relies on strict herd biosecurity, animal isolation, or culling of infected stock to prevent transmission, but these methods are economically damaging.

Scientists investigated a new stem cell gene therapy (GT-CAT1GT\text{-}CAT1GT-CAT1) to treat BLV-positive cattle. GT-CAT1GT\text{-}CAT1GT-CAT1 delivers a gene-editing construct that knocks out the CAT1 receptor gene in hematopoietic stem cells (HSCs).

Scientists conducted a study on two small groups of BLV-positive cattle:

  • Group X (6 cattle) received the stem cell gene therapy along with a pre-treatment of low-dose chemotherapy (busulfan) to deplete existing host hematopoietic stem cells;
  • Group Y (6 cattle) received the stem cell gene therapy following a pre-treatment with targeted monoclonal antibodies (immunotoxins) to deplete host B-lymphocytes;
  • Both groups stopped supportive anti-inflammatory therapy 6 months after starting the gene therapy.

At 18 months after stopping supportive therapy:

  • 4 out of 6 cattle in Group X and 3 out of 6 cattle in Group Y showed complete remission, with no detectable BLV RNA in their blood plasma and no BLV proviral DNA in their B-lymphocytes;
  • B-lymphocytes in the remission cattle did not express the CAT1 receptor;
  • One cow from Group X died from severe bone marrow suppression and opportunistic infection related to chemotherapy during the study.
